Cancellation Policy

I hereby consent to aforementioned person participating in Delco-Cut Enterprises LLC, DBA 941 Talent at Dance University classes and other activities. I acknowledge that potentially severe injuries may occur in any activity involving motion, and other related activities. In consideration for allowing the above student to utilize Dance University’s facilities and for other good and valuable consideration the receipt and sufficiency for which is acknowledged. I hereby forever release, discharge and hold harmless 941 Talent, located at Dance University and it’s owners, shareholders members, managers, directors, officers, employees, independent contractors and instructors from any and all liability for any and all damages and injuries suffered by the above student, while under the supervision, instruction, or control of 941 Talent at Dance University and/or that relate to in any way to the student’s participation in any activities offered by 941 Talent at Dance University. As a student, or legal guardian of the above mentioned student, I hereby agree to individually provide for the possible future medical expenses, which may be incurred by myself or the above student as a result of any injury sustained while training at or performing with 941 Talent at Dance University. This acknowledgement of risk and waiver of liability having been read thoroughly and understood completely is understood completely is signed voluntarily to its content and intent.
